Utilities - Water C/O Hold for Backflow
1. Backflow Prevention (Commercial with Private Fire Line Backflow Ownership or No Fire Line Service) - As provided on the application as a notice, in accordance with the Florida Administrative Code (F.A.C.) Chapter 62-555.360, you are required to install a backflow prevention assembly on all potable, irrigation, and fire lines prior to your activation of your water services. The owner will be responsible for owning, maintaining, and testing all backflow prevention assemblies. A reduced-pressure principle assembly (RP) shall be required on all domestic and irrigation service lines. A double detector check valve assembly (D.D.C.V.A.) must be installed on the dedicated fire line, unless otherwise required in the City's Cross Connection Control and Prevention Manual. You will be required to have the assembly(ies) tested at the time of installation & annually thereafter. You must provide the test results to the Water Department within seven (7) calendar days of test completion. You must install the backflow prevention assembly prior to the inspection to be conducted by the Field Quality Section. Neglecting to install this assembly is a violation of the Florida Administrative Code and enforcement action will occur per F.A.C. 62-550, and your water service will be disconnected when this requirement is not met. A hold has been placed on the Certificate of Occupancy / Certificate of Completion for the final inspection and approval of the installed back flow prevention device(s).
